COVID-19 Community Vaccination Clinics – Important Information for Parents and Guardians

Dear parents and guardians,

 

Your child’s school is participating in a program to transport eligible children to a local COVID-19 community vaccination clinic. This is an important initiative to ensure that your child is protected against COVID-19.

This initiative is optional. Parents and guardians can elect to take their child/ren to GPs, pharmacies or state community vaccination clinics in their own time. For more information about COVID-19 vaccination opportunities visit https://rollup.wa.gov.au/

If you do wish for your child/ren to participate in the school community vaccination clinic, additional information will follow this letter about the date and location of your school’s nominated community vaccination clinic.

Members of the public may also be attending the vaccination clinic at the same time as the students, however the school group will be managed separately within the vaccination clinic and school staff will supervise at all times.

 

What to do:

1. A completed and signed consent form must be provided for every student. A form will be sent home with students. A copy of the consent form is also attached to this notice. All sections of the consent form must be completed. Consent forms must be returned to school by Thursday 21st October.

 2. It is also important that a legal guardian registers their child/children on VaccinateWA . This will ensure that the student’s vaccination record will be received by the Australian Immunisation Register. You can also find further helpful information about registering your dependant here.
